I hereby declare that the information provided in this application is true and accurate to the best of my knowledge. I understand that any false statements may disqualify my application. I recognize that these funds are for emergency purposes and are a one-time opportunity to get assistance to provide lifesaving care for my pet.

I authorize Malta Animal Hospital to my pet’s photo and story in various promotional and informational materials including:

• Website
• Social Media Platforms
• Brochures and Pamphlets
• Newsletters
• Any other promotional materials

The photo and story will be used to highlight the positive outcomes of veterinary care in situations where we are able to utilize CARES Funds.

I understand and acknowledge that the funds awarded are only a portion of the balance for my pet’s care and that payment will be due at the time of services.

Privacy Policy:

CARES Funds respects your privacy. All personal and sensitive information provided in this application will be handled confidentially and used solely for the purpose of assessing your eligibility for assistance.

Note: CARES Funds is committed to helping as many pets as possible, but due to the volume of applications, we may not be able to fulfill every request. Priority will be given to cases of extreme urgency and financial need.
